Achievable

In 2008, Achievable developed a collaboration with the Venice Family Clinic, the largest free clinic in the county, to create "The Achievable Clinic for Individuals with Developmental Disabilities." This clinic uses a "medical home" model of care to deliver quality, comprehensive health care to individuals with developmental disabilities. The advantage of a medical home is that patients gain access to continuous, coordinated care through their primary care provider. The Achievable Clinic is open one afternoon each week to individuals with developmental disabilities who have no other source of care. The Achievable Clinic is staffed by health care providers who have undergone training in treating individuals with developmental disabilities, and are able to provide sensitive, knowledgeable care to our clients. The Achievable Clinic paves the way to increased access to continuous, coordinated care for individuals with developmental disabilities.
